How to Create a PDF Form (Word, Docs, or On Top)

The short answer

Create a PDF form by building it in Word or Google Docs and exporting — Word’s form fields survive the PDF export, and Google Docs auto-turns basic questionnaires into fillable PDFs. If the form already exists as a PDF, add fields on top with a form tool. The honest version: simple forms are easy, complex forms need the right tool.

Build a PDF form in Word (the free route)

  1. Enable the Developer tab (File → Options → Customize Ribbon → check Developer).
  2. Add controls from the Developer tab: text boxes, checkboxes, dropdowns (the Legacy Tools group has the classics).
  3. Write the questions around the fields — “Name: [box]”, “Agree: [checkbox]”.
  4. Export: File → Save As → PDF. The fields ride along.

Build a PDF form in Google Docs

Google Docs has no form-field controls, but its export does something useful: a questionnaire built with plain text and checkboxes converts to a PDF with clickable checkboxes. For very basic forms, that’s genuinely enough — build the questions, File → Download → PDF, and the checkboxes come out interactive. Dropdowns and date pickers need a real form tool.

Forms that start life as PDFs

If the form already exists (a scanned application, a published PDF), you don’t rebuild it — you overlay fields: open it in a tool with form support (Acrobat Pro or an online form builder), click where fields go, save. Details and honest limits are in converting a PDF to a fillable form.

The honest part

Three expectations to set before you build:

  • Recipients need a real reader. Browser viewers sometimes refuse to accept input in fillable forms. Tell people to open the form in Acrobat Reader — free, and the fields always work there.
  • Validation is where forms get real. Required fields, number checks, auto-formatting — Word and Docs don’t do these in PDF export. If the form must enforce rules, you need a proper form builder (paid tools, or a service like Jotform/Google Forms if the data doesn’t need to be a PDF).
  • Signatures are a separate feature. A form that needs signing isn’t a form job, it’s a signing job — plan the flow before you build.
